Does Anyone Have A Simple Form For Drop Ins Or PNO?

I'm doing a parents night out and opened it up to friends/family of my daycare families - so I have 2 little ones coming who are cousins of one of my daycare kids. Wondering if I should have their parents fill out a little form of some sort??? They are going out with my daycare parents so while I will get cell numbers, I know I can contact my dc mom too.

What other info should I get on them?

I currently do daycare from my home PT and work outside the home PT at an after school program. At the after school program, they let kids come for a day to "try it out." They have the parent fill out the first page if their application - name, address, phone, emergency contacts, health conditions and allergies - that's what I can remember off the top of my head.

I suppose it depends on your states regulations. I'm required to have the parents fill out certain forms regardless of whether they'll be regular clients or will only be here for one day oe even one hour (CA). If you are licensed I'd check with your regs first.
