Assistant Performance Record Keeping...WDYD?

So I would like to develop some sort of way of recording my employees performance, be able to go over it with them, and keep it for record keeping for whatever reasons. If anyone is nice enough to share any types of forms or anything like that I would be greatful! Thank you!
