Does anyone have a form they use when they do the terminating of a family ASAP that I could use!?

Does anyone have a form they use when they do the terminating of a family ASAP that I could use!?

I have a form for withdrawal from care but it's usually used for amicable situations.

When it's an immediate termination I just write

DCP

This is written notice of immediate termination of our child care services agreement. No further services will be provided after the date of this notice.

Signed,
No Longer Your Provider

I don't include a reason as 99.9% of the time the parent is fully aware of the issue.
