Emergency Contact Sheet? Incident Report?

I have been looking for an emergency contact sheet that I like and can't find one. I am going to make my own but what kinds of things do you include?

Does anyone happen to have an incident report that I can take a peek at also?

Try your local hospital! ours gives out nice double sided cards with a space for parents information on front, childs name, DOB, allergies. On the back its the consent to treat form. Its the size of a large index card. We laminated them first then wrote on the information so we could reuse them and change information. We stuck a little index card behind them with authorized pick up. We could take these in a fanny pack easily when we go for a walk.
