Permission to Administer Medication Forms?

I used to have one and cannot find a copy. Does anyone have a copy they wouldn't mind sharing? I need one that says I have permission to administer prescription and non-prescription meds. and will notify parent prior to administering. I wish I was better at wording things myself.

Just google Medication administration forms child care...and a TON come up that are free to save/download. I don't know exactly what you want, but some of them look very nice.

Thank you, I did google it and combined 2 forms together into my own. Created one very all-inclusive form out of it and left nothing to question.
I needed one granting me permission to administer topical lotions, etc. plus non prescription medications.
