Accident/Incident Reports

I had a child injured when another child tripped and fell. I can't find any accident reports that I like. Does anyone have one they will share?

(Child A ended up with a bloody nose and split lip, no medical treatment needed. But, after all that Kimberli is going through, we all need to cross our t's and dot our i's.)

Here are a couple different ones. Some are pretty specific and a few are generic so you can change it up and use it however needed. Hope one works for you.

This is the one I use. Pretty much any thing that leaves a mark is documented and given to the parents. You can never be too sure!
