Help with a Form!

Hello everyone. I'm in the process of applying for a license and I need to fill out form LIC401 (Monthly Operating Statement)

I'm not sure what a lot of this lingo means.

What is monthly SSI revenue? Here is the link to the form, if anyone could help, I would appreciate it!

The Supplemental Security Income (SSI) program pays benefits to disabled children who have limited income and resources. Do you have any disabled children that you receive state subsidy for? If so, how many, and how much are you receiving?
