Parent Aware Grants

Tom, I am a provider in MN and received a Parent Aware grant for signing up with the program. They did not send me a 1099 or any type of form in regards to the grant for tax purposes.

I received $500. I purchased an i-pad and the Parent Aware people directly deposited the $500 into my checking account after I sent them the receipt as proof of purchase. How do I claim the money and/or the i-pad?

I asked my other Parent Aware provider-friends and none of them received any type of form from them either...

Report the $500 as income on Schedule C, line 1. Claim the business portion of the IPad on Schedule C, line 18, Office Expense.
