This is my first year and one of my parents just asked me for a statement with my name, address and ss#. I don't have any of that, but is it right, do they need my ss#?

If you have an EIN # do not give out your SS #!!!

They are to provide you with a W-10 form. To cover your own bottom, include the total amount they spent with you on it (there won't be a spot). Make a copy and have them sign the copy for you to keep.
I print mine out from Minute Menu Kids. It already has their total on it and my information filled in. All I have to do is sign it. Much easier for me!
