First Time Doing Taxes - Do I Need Parents' Confirmation?

This is first time doing taxes as a Child Care provider (just run a small home based one). So I was taking some advices from other providers and was calculating how much I received for each child, etc... Then I went and got different statement forms (as I don't really want to use W-10 form unless I am asked to) that people use for sending to parents.

This one form says, "By signing below you are agreeing that the amount listed above is correct. It is extremely important that the amount we both report matches. If you feel there is a discrepancy please inform me before signing. " and signature line. Well... there is a parent whom I would rather not see, the child isn't in my child care anymore anyway, so I was going to send her statement by mail but I doubt she would return the paper signed... She probably doesn't want to see me as much as I don't. Do I really need parents' signature? Can I just say "please contact me back in xx days if there is a discrepancy. Otherwise, you are agreeing that this is the correct amount paid" or anything like that?.. Or would that be illegal to do?

I just really don't want any confrontation... Sigh... I got enough on my plate and the last thing I want is for her to claim that she never signed the paper and come back to me like couple months later and give me trouble for filing my own income tax. (remote, yet possible scenario)

No, you aren't required to have a parent sign your end-of-year receipt. Yes, you can send her a note that says to contact you if the number on the receipt is not correct. That's adequate. If the parent tries to claim that she paid you more than she really did, your records will be more complete than hers, so you will be okay.
