Little screamer

DCG 1.5 years old is quite the screamer and hitter...I don't know what to do anymore! I am not terminating care as she has been here for almost a year and her family is the best, but I can't curb this behavior! I have tried following her around all day to find her triggers, redirecting when an issue occurs or looks like it is ABOUT to occur, I have spoken to the older children about personal space (they always want to be right in her face), I have tried separating her from the older kids, recently I have started time out in a chair like the older kids and she SCREAMS the entire time she is there. I try to ignore it until she calms down but it's so frustrating! We were doing a little better recently (with the screaming at least), then they went on a long weekend away and since she has been back I think she's having a tough time adjusting back to normalcy but I don't think that's 100% of the problem. Her parents and I are definitely on the same page with everything and we are willing to give anything a shot. Please help...I think I might lose my mind!
