Different parent surveys every year?

For those of you that do them(or have to do them Haha), do you create a new one every year? For the past 2 years I gave out the same one. I'm wondering what kinds of questions you ask and if I'm forgetting anything.
If anyone could share their survey here, I'd sure appreciate it.
