    Emergency Info Card

    Does anyone have one, like what the schools have you fill out for your kids? (well....my mom did every year anyway). I'm sure I could create one, but I want something that I can print off on a nice cardstock with the basic information that would be needed in an emergency, that's about the size of a half sheet of printer paper, that i can hole punch and keep on a ring with my evacuation supplies. I just want to see if anyone has one i don't have to make.

    Also...what sorts of information would you put on such a document?

    Mine is not a half sheet, it is a full sheet and it is what the state gives us as a requirement that we have to have. Ill try to attach it here so you can see it. Feel free to use it!

      This is what I used when I had a car big enough to transport children. I found colored cardstock cards that are the size of 1/2 piece of paper.

        I have a double sided half sheet form that is pretty thorough through the state. I don't have it on my computer but I can scan it if you're interested. I do the same thing as you're saying, mine are laminated on a loop ring in my emergency bag.
