Permission to Photograph?

Need help with wording...

I would like to use pictures for crafts and to email to the parents on occasion.

Here is the one I use

My permission to photograph is on my Permission to Participate form.
Each paragraph has a line for parents to initial on the line for Permission Given, or Permission Denied. then sign and date the bottom.



The undersigned parents or guardians of the above named child grant permission for said child to participate in the following daily activities:

Use all play equipment and to participate in all activities presented by the above named Provider which he/she feels is appropriate for the child's age at the time of activity or the use of the equipment.


Leave the Family Child Care home under the constant supervision of the Provider for neighborhood walks or field drips that do not require transportation in a vehicle. Note: Trips that require transportation in a vehicle must have individual signed permission from the parent/guardian.


Use of photographs or videotapes of said child as part of the general program activities and for the Provider's own records.

Mine is much like Crunchy but here it is (only the photography part). Mine is VERY official sounding and there is no way to revoke permission once it is given so that they can't just decide to revoke it and I then have to re-print my advertising materials and remove pictures from my website etc. It also covers written materials, video and audio and has a section of my responsibilities as well and a place for me to sign.

Daycare Name

We, the daycare, sometimes take photographs, audio or video footage for publicity and/or advertising purposes. Also letters, e-mail, and other written material made from you and addressed to us such as letters of recommendations, testimonials etc. may also be used. These may appear in our printed publications, on our websites etc. We may also send them to the news media. Before taking doing so we need your permission. Please read and sign the form.

Photo/Video/Correspondence Consent Form

I, being the parent/legal guardian of _________________________, do hereby consent to the use of photos, video or audio recordings of myself or of my child for [Daycare Name] promotion, including, but not limited to, print, audio, video, and web promotion. I also agree that any writing or other material in connection to [Daycare Name] (including any correspondence from my family) may be used in promotional materials.

I agree that such portraits, pictures, photographs, video and audio recordings shall remain the property of [Daycare Name], unless otherwise noted. In addition, I waive all claims to compensation or damages based on the use of my or my child’s image, correspondence or voice, by [Daycare Name]. I also waive any right to inspect or approve the finished product.

I understand that this consent is perpetual, that I may not revoke it, and that it is binding. I warrant that I am at least 18 years of age and that I am competent in my own name insofar as this consent is concerned. I further attest that I have read this consent form fully and understand it’s contents.

Conditions of Use:
1. [Daycare Name] will not include full names (which means first AND last name) of any person in an image or video on its websites or printed publications.
2. Only images of children that are suitable dressed will be used.
3. Personal information will not be used such as personal e-mail, postal address, and telephone numbers on video, websites or printed publications.
