Sign in/Sign Out Attendance?

Does anyone have a form they would be willing to share? In light of a late pick up I want to start having a sign in/sign out attendance sheet, rather than me filling one in every day.

I PM'ed you mine.

It's in Excel/spreadsheet version so you can enter the names of each child along the left side and enter the correct date at the top.

I enter the names once and leave them but I edit the dates each week as needed. I also black out the columns or spaces in which I am closed or the child is off before placing that sheet out for the week.

Parents sign in and out with times and signatures.
