In the state of Michigan I need a separate form signed for each time I take children in a vehicle unless it is "regularly scheduled" at same time each day such as picking my daughter up from school every day.
My daughter is going to go to band camp this summer for 3 days. I will need to drop her off all 3 days. Do you think this would be sufficient?
